| Subject: | Acro Fields not showing up |
I have created a PDF file, and the field is not showing up on the
page. The text shows up as expected. I thought I got everything
in the right place for the AcroForm, but I could be wrong.
Thank you in advance for any help you can give me.

Thank you for your help, but could you be a little more
specific, or offer a solution?
The annots array is
/Annots[9 0 R] - which points to and object that holds the
array of annots.
Object that has the annots array
9 0 obj
[4 0 R]
endobj
I've removed the reference to the array, and directly pointed
to the annot, but it still doesn't appear on the screen.
The annot is really a Text Field that is part of the AcroForm.

Annots is not supposed to be an array of arrays, but an array of dictionaries/streams. On this basis the (only) PDF you posted was certainly wrong.

I have made the annots a reference to the array, and still the
field isn't showing up.

It looks as if you have made the single most common mistake in PDF creation: misspelling Subtype.
